Prior To Your Initial Consult:
- Contact your insurance company by calling the customer service number on your card and ask if there are benefits for bariatric procedures.
- Complete the new patient packet and return it to our office. Be sure to include a copy of the front and back of your insurance card.
- Attend Dr. May's information session.
Initial Consult:
- You will be seen by our Nurse Practitioner, Kim Mercer.
- Bring all of your current medications with you. Bring the actual bottles, not just a list.
- Bring your insurance card and a complete copy of your medical records.
- You will need to make a co-pay for this visit.
- Bring a list of any questions you may have for the bariatric team.
After Your Initial Consult:
- Our insurance specialist will provide you with a list of forms you will need to complete to be eligible for insurance reimbursement for a bariatric procedure. It is important that you complete everything on the list.
- Set up an appointment for a psychological evaluation.
- Complete all appointments and evaluations requested by our team at the initial consult.
Once All Requirements Have Been Met And Evaluations Received:
- Our team will send a letter to your insurance company to obtain authorization and approval for surgery.
- Legally, your insurance company has 30 days following receipt of our letter to make a decision about your surgery.
Once Approval Has Been Received:
- You will be scheduled for a medical consultation with our bariatric surgeon, Dr. May. At that time, we will schedule your surgery.
- You must attend a mandatory pre-op class prior to surgery. Consult with our Bariatric Surgical Coordinator to determine the correct class for you.
- Two weeks before surgery, you will begin the pre-op diet and education program prescribed by Dr. May.
